Community Nursing Care

Access Care Plus provides professional Community Nursing Care to NDIS participants who require clinical and health-related supports delivered safely within their home or community environment. These supports are designed to assist participants with managing ongoing health conditions, maintaining medical stability, and improving overall health outcomes in line with their individual NDIS plans and clinical requirements.

Our qualified and experienced registered and enrolled nurses deliver a range of nursing supports including medication management and administration, wound care, continence support, catheter and stoma care, diabetes management, monitoring of vital signs, and implementation of care plans as prescribed by medical and allied health professionals. All nursing services are delivered in accordance with best practice guidelines, clinical governance frameworks, and NDIS Practice Standards to ensure safety, quality, and continuity of care.
