Access Care Plus delivers high-quality, person-centred NDIS supports designed to assist participants with a wide range of daily living and personal care needs, while promoting choice, control, dignity, and independence in line with NDIS Practice Standards. Our services include assistance with personal activities at varying levels of support, daily tasks, shared living arrangements, and household tasks, ensuring participants are supported to live safely and comfortably within their own homes and communities.

We also provide tailored support for participants during key life stages and transitions, alongside capacity-building supports focused on the development of essential life skills that enhance independence, confidence, and long-term outcomes. Through assistance with travel and transport, routine planning, and goal-focused supports, we work collaboratively with participants, families, and support networks to enable meaningful participation in everyday activities and support the achievement of individual NDIS goals.

In addition, Access Care Plus offers professional community nursing care and community participation supports, assisting participants to manage health-related needs, attend medical and community appointments, and engage in social, recreational, and community activities.
