
Assist-Life Stage, Transition
Access Care Plus provides specialised support for NDIS participants who are experiencing significant life stage changes or transitions, ensuring continuity of care, stability, and confidence during periods of adjustment. These supports are designed to assist participants as they navigate changes such as moving from school to employment, transitioning between living arrangements, entering independent or supported living, or adjusting to changes in health, care needs, or support networks.

Our team works closely with participants, families, carers, and relevant professionals to develop and implement transition plans that are person-centred, goal-focused, and aligned with each participant’s NDIS plan. Supports may include assistance with daily routines, skill development, coordination of supports, emotional support, and guidance to help participants adapt to new environments, roles, and responsibilities while maintaining their independence, wellbeing, and safety.
